No, Rev: not to difficult: just some basic layering inside photoshop. Myself, I would have tried to tone down the amount of light going into the whipped cream and to add a little light into the cherries and into her outfit. As to the photo inside, the fact that it is darker is good as it adds to the fact that it's in her mind. The inset photo could also be much lighter for the same purpose. I would have photoshopped out the purchase tag front under the table...
